Dr. Pat Taylor is a Family Medicine physician, Hospitalist, and Menopause Society Certified Practitioner. He completed his bachelors in Physiology and Developmental Biology at Brigham Young University, where he played on the National Championship rugby team. He then attended University of North Carolina - Chapel Hill School of Medicine on full academic scholarship. He completed his family medicine residency at University of Utah, where he focused on obesity medicine, men’s health, and menopausal women's health. His research focused on weight management, weight bias, and BMI associated outcomes after joint replacement. Dr. Taylor has a passion for fitness, longevity, and lifestyle modifications; having personally lost 100 pounds after having Cushing’s syndrome and chemotherapy. He previously ran an online weight loss coaching program, and currently continues to share his fitness journey as a busy dad of 6 on social media. In his leisure time, he enjoys golfing, trying new restaurants and patisseries, and spending time with his fiancé and 6 children.
RXFIT BLOOD PANEL ORDER FORM
RXFIT FUNCTIONAL HEALTH GROUP AGREEMENT
Liability Release, Limited HIPAA Authorization, Telehealth & Media Consent
This RxFit Functional Health Group Agreement ("Agreement") is entered into by and between the undersigned participant ("Participant") and Athletic Gym Holdings, LLC, a Utah limited liability company doing business as RxFit Functional Health ("RxFit"), effective as of the date of signature below. In consideration for access to the Functional Health Program described below, the Participant agrees as follows:
1. PROGRAM DESCRIPTION
1.1 Components. The RxFit Functional Health Group ("Program") consists of: (a) a blood draw performed by an independent laboratory; (b) a functional health report compiled by RxFit from the laboratory results and Participant-provided information; (c) six (6) live, remote education sessions via Zoom during which a licensed practitioner presents preventative health concepts, reviews example findings, and answers general questions; and (d) suggests certain health coaching, fitness, nutrition, lifestyle factor, supplements, and medications as part of a treatment plan.
1.2 Not Medical Care. The Program is educational only and does not provide diagnosis, individualized medical advice, or treatment. No provider–patient relationship is formed during the group sessions under this Agreement. If Participant later elects to seek individual treatment from an RxFit Medical Group clinician, that care requires a separate clinical intake, consents, and financial agreement, and is not covered by this Agreement.
2. TELEHEALTH & GROUP PARTICIPATION
2.1 Remote Format. All sessions are delivered via Google Meet or similar secure platform including individual remote meetings with clinician; there are no in-person meetings.
2.2 Group Setting. Participant understands they may be visible/audible to others and controls what they disclose. Participant should avoid sharing sensitive information they prefer to keep private.
2.3 Not for Emergencies. The Program is not urgent or emergent care. For urgent symptoms, Participant will call 911 or seek emergency care immediately.
2.4 Technology Risks. Participant accepts risks inherent in telecommunication (interruptions, delays, poor quality) and will participate from a private setting when possible.
3. LABS, REPORT, AND LIMITED HIPAA AUTHORIZATION
3.1 External Laboratory. Blood draw and testing are performed by an independent laboratory responsible for sample handling, analysis, and reporting.
3.2 Purpose-Limited Authorization. Participant authorizes RxFit and its designated partners to receive Participant’s laboratory results and relevant intake information solely to compile the functional health report and to facilitate Program discussions. Internal sharing is limited to RxFit workforce members involved in Program delivery.
3.3 Revocation & Safeguards. Participant may revoke this authorization in writing at any time; revocation does not affect prior disclosures.
4. EDUCATIONAL CONTENT; NO MEDICAL ADVICE
4.1 General Information Only. All content provided is educational and not a substitute for individualized medical advice. Outcomes vary and no results are guaranteed.
4.2 Encourage Clinical Follow-Up. Participant is encouraged to consult an RxFit Medical Group clinician or their own licensed healthcare provider about personal results and treatment plan decisions.
5. ASSUMPTION OF RISK Participant voluntarily participates and understands risks include: (a) learning of abnormal results that may cause distress; (b) acting on educational information without individualized clinical guidance; (c) inadvertent disclosure of Participant’s own information in a group setting if shared by Participant; and (d) typical telehealth/technology risks.
6. RELEASE OF LIABILITY & INDEMNIFICATION To the maximum extent permitted by law, Participant releases and discharges RxFit and its owners, members, employees, independent contractors (including practitioners serving in an educational capacity), and affiliates from any and all claims, damages, losses, or liabilities arising out of or related to participation in the Program, including reliance on educational content, lab access/transfer, telehealth technologies, or Participant’s voluntary disclosures during group sessions, except where caused by RxFit’s gross negligence or willful misconduct. Participant agrees to indemnify and hold RxFit harmless from third‑party claims arising from Participant’s breach of this Agreement or Participant’s sharing of other participants’ information.
7. PRIVACY IN GROUP SESSIONS; RECORDING Privacy. Participant will not record, screenshot, or disseminate other participants’ images/statements without their written permission.
8. FEES; CASH‑PAY ONLY The Program is cash‑pay only. Fees cover education services, patient blood draw at a third party laboratory, and the functional health report. Clinical treatment, prescriptions, or follow‑up medical care are separate and not included. RxFit does not submit insurance claims or guarantee reimbursement.
9. COMPLIANCE; GOVERNING LAW; SEVERABILITY Services are delivered consistent with applicable federal and state laws; access to certain labs or topics may vary by jurisdiction. This Agreement is governed by Utah law, with exclusive venue in the state or federal courts located in Utah County, Utah. If any provision is held invalid or unenforceable, the remaining provisions remain in full force and effect.
10. MEDIA CONSENT
10.1 Grant of Rights. By participating in the Program, Participant grants Athletic Gym Holdings, LLC dba RxFit Health (“RxFit”), its affiliates, successors, licensees, agents, and assigns, the unconditional, perpetual, worldwide right and permission to record, photograph, film, capture, or otherwise memorialize Participant’s name, likeness, voice, image, statements, and biographical information (collectively, the “Materials”), and to use, reproduce, modify, adapt, publish, distribute, display, and create derivative works from the Materials, in whole or in part, individually or in conjunction with other materials, in any and all media now known or later developed—including but not limited to print, digital, social media, web, podcast, broadcast, and internal training materials—for any lawful purpose related to RxFit’s marketing, advertising, educational, or internal communications.
10.2 Ownership and Waiver. All rights, title, and interest in the Materials shall belong exclusively to RxFit. Participant waives any right to inspect or approve the finished product or any use thereof, and waives all claims for compensation, royalties, or additional consideration arising from the use of the Materials.
10.3 Irrevocable and Perpetual Release. This grant is irrevocable, perpetual, and worldwide, and applies without limitation as to time, medium, or geography. RxFit is under no obligation to use the Materials but may do so at its sole discretion.
10.4 Release of Claims. Participant releases and holds harmless RxFit and all persons acting under its authority from any and all claims, demands, or causes of action arising out of or related to the creation or use of the Materials, including but not limited to claims for defamation, invasion of privacy, or violation of rights of publicity.
10.5 Lawful Use Limitation. Nothing in this section permits alteration or use of the Materials in a defamatory, unlawful, or misleading manner. RxFit agrees to use the Materials solely in lawful and professional contexts consistent with its brand standards.
